Welcome to our "Agencies Only" page. This page is designed to help caseworkers access Helping Hearts & Hands (HHH) assistance for the individuals and families whom you serve. It is our goal to make the application process as smooth and quick as possible. We understand that most clients are in urgent situations and need help immediately.

Applications cannot be submitted online, as they require a signature. Please be sure that they are filled out completely; they may be faxed to (303) 339-6463 or scanned and emailed to HHH Applications (helpingheartsandhands@jeffcoac.org) by the referring agency. Applications are not accepted directly from clients.
